Whether you’re just starting out or are already caring for someone, here’s a practical guide to help you navigate the system and make sure your loved one gets the care and support they deserve.


1. Get a Great GP (and the right diagnoses)


A proactive, supportive GP can make a big difference.

  • Don’t hesitate to switch to a better practice if needed - it will be worth the effort

  • Request a full medication review at least once a year

  • Ask for formal diagnoses to unlock the right services

  • Request referrals to specialists e.g. memory clinic, bladder & bowel services, nutritionists etc.



2. Get ‘Known’ to the System


Request a Care Act Assessment from your council - this can open the door to funded support:

3. Know Your Rights – and Use Them


Even if your care is funded by the council, you don’t have to settle for what’s offered. 

Ask for a Personal Budget or Direct Payments to give YOU choice about your care. This means you can choose your own care provider – like Sirius Homecare. 




4. Get Essential Paperwork in Place – Now


Prepare for the future while your loved one can still make decisions. 

  • ✅ ReSPECT form (Recommended Summary Plan for Emergency Care & Treatment) – ask your GP 🔗 ReSPECT Info 

  • Lasting Power of Attorney (Health & Welfare and Finance) - allows your loved one to appoint someone they trust to make decisions on their behalf if they lose the ability to do so themselves - ensuring their wishes are respected and their affairs are managed as they intend 

  • A Will – to ensure their wishes are followed



5. Take Care of Yourself Too
